Federal investigators say a misunderstanding between a pilot and co pilot of a United Airlines flight in December led to the plane diving within 748 ft of the ocean. The flight was traveling from Maui Hawaii to San Francisco. The National Transportation Safety Board's report found after takeoff, the captain asked the co pilot to reset the wing flaps. Reportedly the co pilot heard 15 instead of five, the mix up caused the crew to mismanage the plane's path, air speed and nose direction. The plane reportedly plunged more than 1400 ft towards the ocean before the pilots were able to make a recovery and proceed to their final destination. There were no injuries or even any damage to the plane.
