A new report found errors and failures at a Manhattan jail that led to the death of disgraced financier Jeffrey Epstein who died by suicide in 2019. The US Justice Department report revealed Epstein died due to severe negligence and mismanagement inside the metropolitan correctional center in New York City was shut down in 2021 because of its deteriorating conditions. The report found guards failed to conduct rounds to not monitor Epstein and later falsified jail records to cover up their misconduct. Epstein was at the federal prison awaiting his trial on sex trafficking and conspiracy charges.
